The patient has the ability to edit and update their personal settings and information held about them on the platform. 


The Edit Patient page is a replica of the create patient form where a patient can update their information, this information then updates the patients summary record to ensure all information is kept accurate and up to date. 


The patient can edit the following information:

 Contact Information

  • First Name
  • Last Name
  • Gender 
  • Gender at Birth 
  • Date of Birth 
  • Mobile Number 
  • Post Code 
  • Address 
  • Email Address 
  • Landline Number 
  • Contact Preference 
  • Communication Requirement 

 GP Details

  • GP Name 
  • GP Practice 
  • GP Practice Address 

Demographics

  • How would you like us to refer to you? 
  • Language Preference 
  • Country of Birth 
  • Ethnicity 

Communication Preferences

  • Email communications 
  • SMS communications 
  • Postal letter communications 
  • Consent to email and SMS reminders
  • Consent for GoJ to send letters to home address


Note: Patient Identifiers have been removed from the Edit screen and no ability is given to the patient to update their Identifiers for security purposes.
